Role Overview: Care CoordinatorAs a Care Coordinator, you will be a key member of the team, ensuring that our clients receive the highest standards of care. Your responsibilities will include:
- Coordinating care schedules and ensuring effective allocation of resources.
- Maintaining accurate and up-to-date client records and care plans.
- Supporting the recruitment, training, and ongoing development of care staff.
- Liaising with service users, their families, and healthcare professionals to ensure care needs are met.
- Monitoring the quality of care provided and conducting regular checks and reviews.
- Handling client queries and complaints, ensuring that all issues are resolved in a timely and professional manner.
- NVQ/QCF Level 2 or Level 3 Diploma in Health and Social Care or equivalent experience.
- At least 2 years of experience in the health and social care sector.
- Strong organizational and communication skills.
- A Full UK driving license and access to your own vehicle.
- Ability to manage multiple tasks and coordinate care effectively.
- A proactive approach to improving customer satisfaction.
- Ability to build and maintain effective relationships with staff, clients, and stakeholders.
- Good IT skills and experience with care management software is a plus.
